The Make-A-Wish Foundation ® of the Tri-Counties grants the wishes of children with life-threatening medical conditions to enrich the human experience with hope, strength and joy. We serve Ventura, Santa Barbara and San Luis Obispo Counties.

Join us for a celebration of wishes as well as silent and live auctions and Fund-A-Wish in the Monk Room followed by special seating for the musical acts that evening.

*SCHEDULE*
5:45 doors open
5:45 – 7:10 silent auction
5:45 - 6:15 pre-order dinner (purchased separately)
6:30 – 7:45 Celebration of Wishes, Live Auction and Fund-AWish
7:45 - dinner served
8:00 - Jake Shimabukuro - ukelele virtuoso
8:15 - 9:15 baskets available for pickup

Jake Shimabukuro is a ukulele virtuoso known for his complex finger work. His music combines elements of jazz, rock and pop. Jake’s music appeals to traditional fans of Hawaiian music, to jam band fans, to today’s surf rock music scene.

The Canyon Club has donated tickets so 100% of your ticket price benefits the Make-A-Wish Foundation of the Tri-Counties. The fair market value of a ticket is $22/ticket based on the value of Jake Shimabukuro tickets ($22/ticket plus $8/ticket fee for purchase on-line). Admission includes Wish Night activities and admission to all entertainment that night.

*WHAT IS FUND-A-WISH?*
Become part of the magic and help create more heartwarming stories though Fund-A-Wish, a donor option offered only at Wish Night where donors fund a wish or a portion of a wish.
• $5,000 funds a full wish
• $2,500 funds ½ of a wish
• $1,000 funds 1/5 of a wish
• $ 500 funds 1/10 of a wish
Some of the 75+ wishes in progress will be included in your Wish Fan which you will receive at the check in table. Bidding will start with full wishes. The first Fund-A-Wish donor will select a wish and then bidding will continue with the remaining wishes available, moving through the funding levels. You may combine funds with friends to fund a wish at any level. Fund-A-Wish sponsors will receive a certificate immediately denoting their wish, a tax letter by mail, and photos and updates on the wish by email.

A 5% on-line ticket administration fee will be added to your bill. Those purchasing tables may order tables by mail and avoid the on-line ticket fee. Please send your ticket order with a check to Make-A-Wish Foundation of the Tri-Counties, 4222 Market Street, Suite D, Ventura CA 93003 by February 2,
2012. Please include your phone number and e-mail so we can contact you to confirm your order.

Dinner and drinks are purchased separately. For all shows at the Canyon Club purchase of food is required for table seating. When you arrive you will be provided with a menu which will include appetizers, dinners and the child’s menu. Place your orders between 5:45 and 6:15 using the order form. Our tax identification number is 77-0098671. Gifts to the Make-A-Wish Foundation of the Tri-Counties are tax deductible as provided by law. Fair market value of the evening is $20/person. Tickets have been donated by the Canyon Club.
